Spring 2021 COVID-19 Form

As a registrant of our Spring Programming, each child must have on file a completed COVID AGREEMENT form prior to stepping on the field during our Spring Programming for 2021.

There is no need to bring a copy of this form to the field.  The agreement stipulates each parent/guardian will do a Health Assessment of the player prior to bringing them to the field for each session.  Attendance will be taken at the field to ensure contact tracing is enabled.

This form is a Provincial Health Order requirement that is also part of our Field Use Agreement with the City of Abbotsford.

• If the player is not well, keep them at home.
• All sections of the form are required.
